The patient may be scared of being in the closed setting, or may not understand why they are in the closed setting. In the first instance, use behaviour management strategies to address difficult behaviour (Table 2). Physical exercise may prolong withdrawal and make withdrawal symptoms worse. Patients who are opioid dependent and consent to commence methadone maintenance treatment do not require WM; they can be commenced on methadone immediately (see opioid withdrawal protocol for more information). Protracted withdrawal is a long-term withdrawal syndrome that may come and go for several months.
Tapering off supratherapeutic doses
Those respondents taking a full dose of benzodiazepine tended to the lowest rates of adverse life consequences. 7. WITHDRAWAL MANAGEMENT FOR INHALANT DEPENDENCE
Table 3 provides guidance on medications for alleviating common withdrawal symptoms. The survey asked about 23 specific symptoms and more than half of the respondents who experienced low energy, distractedness, memory loss, nervousness, anxiety, and other symptoms stated that these symptoms lasted a year or longer. These symptoms were often reported as de novo and distinct from the symptoms for which the benzodiazepines were originally prescribed. A subset of respondents stated that symptoms persisted even after benzodiazepines had been discontinued for a year or more.
- In this case report, a 27-year-old male with a history of benzodiazepine and opioid abuse presented to our emergency department with a chief complaint of drug withdrawal.
- But if you remove the drug all at once, your brain doesn’t have enough time to prepare, and you can develop life-threatening symptoms like seizures.
- In the first instance, attempt behavioural management strategies as shown in Table 2 (page 33).

How Long Does Withdrawal From Benzodiazepines Last?
This helps to relieve benzodiazepine withdrawal symptoms and prevent the development of seizures. Short-acting benzodiazepines include oxazepam, alprazolam and temazepam. Withdrawal severe benzodiazepine withdrawal syndrome typically begins 1-2 days after the last dose, and continues for 2-4 weeks or longer. Codeine phosphate alleviates opioid withdrawal symptoms and reduces cravings.
- If you have been using benzodiazepines for longer than six months, suddenly stopping your dose can cause grand mal seizures and delirium—this is why it’s best to involve your doctor or healthcare professional in your withdrawal process.
- Buprenorphine is the best opioid medication for management of moderate to severe opioid withdrawal.
- Currently, ketamine is FDA-approved for anesthesia and sedation but has off-label indications for chronic pain management and post-traumatic stress disorder (PTSD).
- BIND serves as a clinically serviceable name for the enduring neurological sequelae of benzodiazepine use and would reify this condition for healthcare professionals.
Management of benzodiazepine withdrawal
Estimates suggest that 10 to 25% of people who take benzodiazepines for extended periods experience what’s known as protracted withdrawal. Longer-acting benzodiazepines like Klonopin (clonazepam) can stay in the system longer, which means it can be one to two days or even longer before withdrawal symptoms start. When you are physically dependent on a drug, it means your body can’t operate normally without it. If you stop or reduce your dose suddenly, you will experience withdrawal symptoms.
Signs and symptoms
Withdrawal symptoms from short-acting drugs, such as Xanax, may come on faster than withdrawal symptoms from long-acting drugs, such as Valium. Benzodiazepine withdrawal can be an unpleasant and challenging aspect of early recovery. If not medically managed, some of the symptoms of benzodiazepine withdrawal may be severe and potentially dangerous.
Tapering changes
Patients should drink 2-3 litres of water per day while in withdrawal. Offer symptomatic medication as required for symptoms such as headaches, nausea and anxiety (Table 3). Give 20mg diazepam by mouth every 1-2 hours until symptoms are controlled and AWS score is less than 5.
